Requirements

  • Master's Degree in Social Work (MSW) required.
  • Current state social work licensure required.
  • Three (3) years of social work experience in an acute care setting required.
  • Experience facilitating advance care planning and goals-of-care discussions required.
  • Experience performing psychosocial assessments and supporting patients with serious illness required.
  • Serious illness communication and family meeting facilitation.
  • Psychosocial assessment, counseling, grief and bereavement support.
  • Knowledge of hospice eligibility and community resources.
  • Ability to identify surrogate decision makers and support healthcare proxy designation.
  • Strong interdisciplinary collaboration skills.
  • Ability to effectively educate medical students and residents.
  • Strong documentation, organizational, and communication skills.

Nice To Haves

  • Licensed Clinical Social Worker (LCSW) preferred.
  • Coursework or training in Hospice and Palliative Care preferred.
  • Coursework or training in Advance Care Planning preferred.
  • Advanced Palliative and Hospice Social Work Certification (APHSW-C) preferred.
  • Experience facilitating family meetings and ethics consultations preferred.

Responsibilities

  • Conduct independent psychosocial assessments.
  • Provide counseling, grief support, loss support, crisis intervention and bereavement support.
  • Assess caregiver burden and develop support plans.
  • Introduce patients and families to palliative care services.
  • Navigate healthcare systems and community resources.
  • Advocate for patients and families.
  • Document assessments and interventions in the medical record.
  • Facilitate advance care planning discussions.
  • Participate in goals-of-care conversations.
  • Identify surrogate decision makers.
  • Assist with Advance Directives, Durable Power of Attorney, and POLST/MOLST (Physician/Medical Orders for Life-Sustaining Treatment) completion.
  • Coordinate and schedule family meetings.
  • Participate in ethics consultations.
  • Educate patients and families regarding palliative care and hospice services.
  • Coordinate discharge planning for palliative care patients.
  • Assist with hospice, home-based palliative care, long-term care, and post-acute placement.
  • Participate in interdisciplinary rounds and care planning.
  • Support education of residents and medical students.
  • Track quality and program metrics.
