About The Position

This role is responsible for providing psychosocial-emotional support and services to patients and families. The Social Worker will assist patients and families in understanding, coping, and adapting to the personal, emotional, and bio-psychosocial issues associated with the patient's illness. This includes conducting comprehensive assessments, making referrals, providing crisis intervention, and managing care. The position requires collaboration with a multi-disciplinary team and other internal and external personnel to promote awareness of environmental and psychosocial needs, and to develop and implement treatment plans that support a patient-centered plan of care for both individual patients and the medical community. The role may also involve providing functional direction to unlicensed social work staff and social work interns.

Requirements

  • Master's Degree in Social Work required
  • 1 year of clinical, case management, social work or related experience required
  • Current Basic Life Support for Healthcare Providers by American Heart Association required
