As a Social Worker with WesleyLife At Home, you will explain WesleyLife Hospice and Palliative services to patients and families. You will visit patient homes to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers. You will communicate findings to patient care teams to update care plans, carry out social evaluations, and plan interventions. You will counsel patients and families/caregivers regarding stress and other identified coping difficulties, and provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs. You will serve as a liaison with patients and families/caregivers and care providers or community agencies. You will also participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care.
