Our Level 1 acute care hospital serves adults and adolescents with a wide range of psychiatric and chemical dependency needs. We are seeking a licensed or license-eligible Social Worker/Therapist to join our Adolescent Unit as a Patient Care Manager (PCM) at our East facility. This is a full-time position working primarily with adolescents in an inpatient psychiatric setting. The ideal candidate thrives in a fast-paced environment, is passionate about working with youth, and values teamwork and quality patient care.
Requirements
- Master's degree in Social Work, Counseling, or related field
- Must be at least 21 years of age
- Ability to pass background check, drug screening, and obtain fingerprint clearance
Nice To Haves
- Licensed or license-eligible in Arizona (LAC, LPC, LMSW, LCSW, LMFT, LISAC)
- Candidates working toward licensure (MSW, MS, LMSW, LAC, LASAC) will be considered
- Minimum of 2 years of experience in a behavioral health setting
Responsibilities
- Manage and coordinate care for an assigned adolescent patient caseload
- Provide group therapy and psychoeducation
- Complete accurate and timely clinical documentation
- Collaborate with interdisciplinary team members to support patient outcomes
- Participate in utilization review processes
- Deliver excellent customer service to patients and families
