About The Position
The Social Worker will be responsible for organizing, developing, and coordinating aspects of resident care. This includes performing initial and ongoing assessments to meet resident needs and serving as a liaison between the resident, family, and staff regarding any concerns that may arise.
Requirements
- Must be willing to work flexible hours.
- Must have excellent communication skills, written and verbal.
- LSW or a degree in social work required, a degree in similar field with experience on long term care will be considered.
Nice To Haves
- At least one year experience in Social Service work in a long term care setting preferred but not necessary
Responsibilities
- Organizing, developing and coordinating aspects of resident care.
- Initial and ongoing assessments required to meet resident needs.
- Liaison between the resident, family & staff regarding concerns, which may arise.
