About The Position

Provides critical psycho-social support to Palliative Care patients and their families through the Palliative Care consult team. Utilizes professional background and skills to assist the team and facilitate patient-family meetings. Assist team, attending physicians, staff, and patient and family in the clarification of the goals of care and the plan of care.

Requirements

  • Master's degree in Social Work from an accredited school.
  • Three (3) years of experience in a health-related area.
  • Two (2) years working with the frail or elderly population.
  • Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker (LISW)
  • A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.
  • Individual and family therapy skills
  • Analytical thinking and problem-solving abilities.

  • Team members employed in this job prior to July 1, 2020, are grandfathered under prior educational and experience requirements.
  • Team members must be certification eligible and will have one year to obtain required certification.

Responsibilities

  • Participates in the Multidisciplinary Team’s initial assessments, care planning and periodic re-assessments. Assessments are to be completed prior to the scheduled team meetings.
  • Communicates patient changes to team members.
  • Maintains current written case management records both in paper and electronic format as required, including periodic reassessments of program participants.
  • Provides individual and family counseling, develops and leads group counseling and activities.
  • Coordinates with Discharge Planner and Case Manager to ensure care coordination and appropriate assignments and resource allocation to meet patient needs throughout the continuum of care.
  • Coordinates team efforts with appropriate hospital support departments to ensure smooth discharge planning and clear follow up post discharge; may assist in the development of creative community or system care options for patients.
  • Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
  • Participates in Program, Policy and Quality Improvement activities of Social Work component.
  • Acts as spokesperson for the Palliative Care Program at the hospital’s discretion, to enhance the Palliative Care Service’s community outreach to physicians, the public, healthcare facilities and other applicable organizations in the community.
  • Performs other duties as assigned.
