Medical Social Worker (FT Days)
About The Position
The Social Worker role provides coverage under Ohio's Hospice South regions. This is a full-time position, Monday-Friday, 8 AM-5 PM, with after-hours/weekend on-call duties. The organization is dedicated to providing superior care and services to patients at their end-of-life journey. The Social Worker will utilize their advanced educational background, clinical expertise, judgment, and discretion to identify the patient and family's unique cultural/spiritual beliefs, coping skills, pre-bereavement, and anticipatory grief needs, offering a wide range of services to terminally-ill patients and families in homes and facilities.
Requirements
- Licensed Social Worker with the state of Ohio.
- Master's Degree in Social Work or related degree as required for LSW licensure.
- Computer skills sufficient to properly document services and care.
- Able to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance coverage.
- Notary designation or intention to complete designation within six months of hire.
Nice To Haves
- LISW preferred or LSW.
- One year social worker experience in a health care setting preferred.
- Previous discharge planning experience.
