About The Position

The Social Worker MSW supports participants and their families with personal and environmental difficulties that predispose illness or interfere with remaining in their home environment. The role applies analysis, interpretation, and interpersonal skills to help patients navigate complex issues. This position is in Emergency Department Case Management at MultiCare Yakima Memorial Hospital and is scheduled for 1.0 FTE, evenings, five 8-hour shifts from 5:00 p.m. to 1:30 a.m. The posting offers a $10,000 sign-on bonus with a 24-month commitment.

Requirements

Master’s degree in Social Work.
Hold an LSWAIC, LMHC, LMFT, LASW, LICSW, or AAC credential as required by the department.
Applicants who have submitted a completed associate license application to the Washington State Department of Health may practice under supervision for up to 120 days while the application is pending.
Experience working in a hospital or acute-care setting, or an equivalent social service agency, with experience relevant to the medical social work function.
BLS certification may be required depending on the assigned area.

Responsibilities

Administer assessment tools and gather assessment data to determine potentially needed services.
Interview families to determine participants’ social work needs.
Contact community resources to coordinate services.
Provide initial and ongoing assessment of participants’ social, emotional, and cognitive status and needs, involving participants and other staff members in the assessment.
Maintain accurate documentation for assessments, treatment plans, and evaluations.
