About The Position
The Social Worker MSW supports participants and their families with personal and environmental difficulties that may predispose them to illness or interfere with remaining in their home environment. The role uses analysis, interpretation, and interpersonal skills to help patients navigate complex issues. The position is in inpatient case management at MultiCare Yakima Memorial Hospital.
Requirements
- Master’s degree in Social Work.
- Hold a credential required by the department: LSWAIC, LMHC, LMFT, LASW, LICSW, or AAC. Individuals who have submitted a completed associate license application to the Washington State Department of Health may practice under supervision for up to 120 days while the application is pending.
- Experience in a hospital or acute-care setting, or an equivalent social service agency, with experience relevant to the medical social work function.
- BLS certification may be required depending on the assigned area.
Responsibilities
- Administer assessment tools and gather data to assess for optional needed services.
- Interview families to determine participants’ social work needs.
- Contact community resources to coordinate services.
- Provide initial and ongoing assessment of participants’ social, emotional, and cognitive status and needs, involving participants and other staff members in the assessment.
- Maintain accurate documentation of assessments, treatment plans, and evaluations.
