About The Position

Horizon Health Corporation is seeking a Social Worker (MSW) for our BRAND-NEW unit called Altru Behavioral Health Center in Grand Forks, ND. This unit will be a 24-bed inpatient unit for both adult and pediatric patients. We are also planning to expand to an additional 24 beds in 2026. Altru Health System and Universal Health Services are working together in expanding inpatient behavioral health services. The Social Worker is responsible for providing direct high quality psychotherapy services to patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. The Social Worker assists patients and their families in understanding and adjusting to diagnoses and potential treatments. Social Worker can also help identify resources during treatment and after discharge.

Requirements

  • Master's Degree (Master of Social Work - MSW) required.
  • Minimum five years' experience
  • Completes competent, clear progress notes and assessments according to program policies and procedures
  • Crisis prevention experience
  • Ability to manage anger, fear hostility and violence of others
  • Ability to manage stress appropriately
  • Proficient in applicable software applications
  • Excellent interpersonal skills
  • Communicates effectively with other professionals and support staff to achieve positive patient outcomes
  • Above average organizational skills
  • Ability to prioritize multiple tasks

Responsibilities

  • Designs and implements a treatment plan of care for patients based on a thorough assessment of the patient’s symptom presentation, professional/referral input and all other available data.
  • Participates in referral development processes and is an active member of the referral development team.
  • Actively involved in assessing patient progress and communicating results and trends to supervisor and treatment team.
  • Conducts individual and/or psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques.
  • Provides appropriate education, resources and support for patients before, during, and after treatment.
  • Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources and makes effective contact with disposition sources.
  • Completes progress notes and assessments according to program procedures.
  • Actively involved in measuring patient performance; communicates results and trends to supervisor. Provides feedback to supervisor on an ongoing basis in regard to concerns, improvements, changes, etc.
  • Participates as an active member of the interdisciplinary and treatment teams.
  • Performs other duties as assigned/required by this position.
