About The Position

Under the supervision of the Social Services Manager, this role plans, organizes, and implements social work services for INSPIRA LIFE participants and their families. Responsibilities include assessment, treatment, teaching, and counseling. The Social Worker acts as a liaison between the interdisciplinary team, caregivers, and community agencies. This role also involves completing NJ Choice Level of Care (LOC) Assessments and ensuring QA compliance. As a multisite provider, employees may work at either the Vineland or Williamstown LIFE Program locations based on operational needs.

Requirements

  • Master’s degree in social work from an accredited school of social work.
  • Minimum of 1 year working with the frail or elderly required.
  • Licensed with the New Jersey Board of Social Work required.
  • NJ State Choice Assessment Certification to complete level of care assessments (can be obtained within the first 6 months of employment).

Nice To Haves

  • 2 years’ experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
  • Experience with Life or PACE program preferred.
  • Member of the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification preferred.

Responsibilities

  • Plans, organizes, and implements social work services to INSPIRA LIFE participants and families.
  • Conducts assessments, treatment, teaching, and counseling for participants, caregivers, or their representatives.
  • Performs individual participant contacts and appropriate collateral contacts.
  • Provides participant and family education, assessment, and counseling.
  • Offers resources and addresses mental health needs as they arise.
  • Manages ongoing case management.
  • Advocates to ensure participant and caregiver needs are met.
  • Handles disenrollment procedures.
  • Serves as the liaison between the interdisciplinary team, caregiver representatives, and community agencies.
  • Completes NJ Choice Level of Care (LOC) Assessments initially and annually, ensuring QA compliance.
