About The Position

The Social Worker MSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation. The Social Worker MSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.

Requirements

  • Master’s of Social Work or Bachelor’s of Social Work with Master's Degree in related field.
  • Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintained.
  • One (1) year of previous social work experience.
  • Basic computer skills including knowledge of MS Office – Outlook and Word.
  • Current knowledge of healthcare economic issues impacting acute care.
  • Working knowledge of social work scope of practice and current issues impacting healthcare.
  • Understanding of payer/reimbursement practices and regulations.
  • Ability to understand and comply with Federal and state regulatory standards of care.
  • Ability to effectively handle multiple demands in a short timeframe.

Nice To Haves

  • CCM or ACM Case Management Certification.
  • Case Management experience in a hospital setting.

Responsibilities

  • Assists with coordination of referrals for post-acute services as appropriate in a timely manner.
  • Effectively communicates with the patient/caregiver and physician on payer or other financial issues.
  • Coordinates and documents complex discharge plans.
  • Collaboratively determines desired discharge goals and communicates all discharge plans and options to patient/family, physicians and staff.
  • Participates in patient care conferences/ethics consults as indicated.
  • Provides counseling and researches placement alternatives for substance abuse, mental health, homeless and indigent patients.
  • Facilitates and participates in transfer of Baker Act and Marchman placements to mental health facilities.
  • Provides therapeutic interventions with patients and families as needed.
  • Completes mandatory reporting processes.
  • Facilitates referrals for post-discharge psychosocial, therapeutic, and any other needs as assigned.
  • Identifies system/process breakdowns and improvement opportunities.
  • Communicates avoidable delays utilizing the escalation process to remove barriers to progression of care.
  • Ensures compliance with regulatory standards, identifying risk management quality issues and reporting as required.
