About The Position
Provides advanced clinical and psychosocial support to high-risk, medically complex patients. Responsible for assessment, care coordination, discharge planning, crisis intervention, and addressing social determinants of health. Assists in transitional care management as needed. Promotes patient outcomes, supporting value-based care, and reduction in avoidable readmissions.
Requirements
- Masters or Doctoral Degree in Social Work from an accredited program
- Proficiency in completing assessments, treatment planning, aftercare planning and crisis management
- Exceptional communication skills with the ability to interact professionally with professionals of multiple disciplines
- Minimum of 5 years’ clinical case management experience
Nice To Haves
- Licensure in Social Work in the state of Kentucky
- Certified Social Worker license by Kentucky Board of Social Work (KBSW)
