About The Position

Under the supervision of the Social Services Manager, this role plans, organizes, and implements social work services for INSPIRA LIFE participants and their families. Responsibilities include assessment, treatment, teaching, and counseling for participants, caregivers, or their representatives. Interventions may involve individual participant contacts, collateral contacts, participant and family education, assessment, and counseling, provision of resources, addressing mental health needs, ongoing case management, advocacy, and disenrollment procedures. The Social Worker serves as a liaison between the interdisciplinary team, caregiver representatives, and community agencies. This role also includes completing NJ Choice Level of Care (LOC) Assessments initially and annually, ensuring QA compliance. As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, employees may work at either the Vineland or Williamstown LIFE Program locations based on census changes or operational needs.

Requirements

  • Master’s degree in social work from an accredited school of social work.
  • Minimum of 1 year working with the frail or elderly required.
  • Licensed with the New Jersey Board of Social Work required.
  • NJ State Choice Assessment Certification to complete level of care assessments (this certification can be obtained within the first 6 months of employment).

Nice To Haves

  • 2 years’ experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
  • Experience with Life or PACE program preferred.
  • Member of the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification preferred.

Responsibilities
