About The Position

We are seeking a dedicated Social Worker Mental Health Consultant to join our team at Hickam Air Force Base (Joint Base Pearl Harbor-Hickam), Hawaii. In this role, you will provide expert clinical guidance, shape preventative mental health programs, and directly impact the operational readiness and well-being of our Airmen, service members, and their families stationed in the strategic Indo-Pacific theater. The Hickam AFB Medical Facility, officially known as the 15th Medical Group, is located at 755 Scott Circle (Bldg 559), Hickam AFB, HI. It operates as an outpatient clinic for active-duty members and their families.

Requirements

U.S. Citizenship required.
Master of Social Work (MSW) from a CSWE-accredited institution.
Active, unrestricted Clinical Social Work License (e.g., LCSW, LICSW) permitting independent practice from any U.S. jurisdiction.

Responsibilities

Conduct brief, targeted biopsychosocial assessments.
Deliver evidence-based interventions in 30-minute appointments using a stepped-care model.
Treat chronic conditions, insomnia, acute stress, and health behaviors.
Triage/refer complex cases to Specialty Mental Health.
Complete concise electronic medical record documentation within 72 hours of the visit.
Provide immediate, succinct feedback to Primary Care Managers (PCMs).
Actively drive same-day "warm handoffs" and increase clinic-wide service penetration.
Review daily PCM schedules ("scrubbing lists") and participate in morning huddles.
Offer continuity consultation to monitor long-term patients and prevent relapse.
Develop and lead educational classes focused on coping skills and behavior change.
Train primary care clinic staff to recognize symptoms and optimize PCMHI referral pathways.
