About The Position

William Osler Health System (Osler) is seeking a Social Worker for their Mental Health Geriatric program. This role involves providing direct social work practice, including psychosocial assessments, individual, marital, and family therapy, and facilitating psychoeducational and process groups. The social worker will also consult and coordinate patient care within the hospital and collaborate with community resources, complete necessary documentation, and participate in ongoing professional development. This position is primarily based at Brampton Civic Hospital with some work at Peel Memorial Centre for outpatient referrals.

Requirements

  • Masters of Social Work
  • Registration with the O.C.S.W.S.S.W. and in good standing
  • Minimum of 3-5 years social work experience in Mental Health
  • Experience in developing and conducting psychosocial and process groups
  • Demonstrated experience in providing psychotherapy including family therapy
  • Excellent psychosocial assessment and intervention skills including crisis intervention
  • Demonstrated understanding of serious & persistent mental illness, concurrent disorders and its presentation in the geriatric population
  • Familiarity with community resources and demonstrated ability to work well with community partners
  • Experience in the treatment of mood and anxiety disorders and other complex mental health issues
  • Demonstrated excellent organizational, interpersonal and communication skills.
  • Must be able to work independently and interdependently within multidisciplinary treatment team
  • Computer proficiency with Microsoft Word, PowerPoint, Outlook, Meditech, and Epic
  • Excellent attendance, punctuality and work record

Responsibilities

  • Provide psychosocial assessments for individuals referred from within the hospital and the community.
  • Conduct individual psychotherapy, marital, and family therapy.
  • Plan and conduct psychoeducational and process groups.
  • Facilitate an evening family group.
  • Consult and co-ordinate patient care within the hospital.
  • Collaborate with community resources.
  • Complete forms and letters as required for patient care and discharge planning.
  • Follow documentation guidelines as defined by the social work college and the mental health department.
  • Participate in ongoing professional development.
