About The Position

Anoka County Community Social Services and Behavioral Health is committed to meeting people’s needs through quality services. We are seeking a Social Worker to join our Adult Mental Health team. This position provides case management services to adults with serious and persistent mental illness, including individuals under civil commitment for whom the county is mandated to provide support under federal and state requirements. The role focuses on coordinating access to mental health services and community resources that help individuals remain safely in their homes and communities. The Social Worker will manage a dedicated caseload and work closely with adults and their families to ensure their mental health needs are effectively addressed. Requirements

  • Bachelor’s degree in Social Work or a closely related field.
  • Two years (2,080 annual hours) of job-related experience working directly with adults with serious and persistent mental illness.
  • In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.
  • Valid driver’s license and vehicle available for business use.

  • Master’s degree or higher in Social Work or a closely related field.
  • Three or more years (2,080 annual hours) of job-related experience working directly with adults with serious and persistent mental illness.
  • Experience working within SSIS database.
  • Strong verbal and written communication skills.

Responsibilities

  • Maintain caseload comprised of adults with serious and persistent mental illness with severe emotional disorders.
  • Integrates general knowledge of mental health and substance use disorder into the creation of person-centered service plans and optimal use of community resources.
  • Support families as they strive to understand and meet the mental health needs of their adult family member.
  • Evaluate and screen maltreatment reports to determine need for Emergency Protective Services.
  • Identify people and communities in need of help.
  • Evaluate clients (existing, new and prospective) based on their needs, limitations and desires.
  • Assess clients’ needs, situations, strengths, and support networks to determine their goals.
  • Build rapport with clients and their families.
  • Assist clients in receiving services by telephone and in person.
  • Help clients adjust to changes and challenges in their lives.
  • Monitor clients' situations and follow up to ensure that they have improved.
  • Advocate or raise awareness with and on behalf of their clients.
  • Timely completion of paperwork and case notes in SSIS, maintain electronic files in OnBase, as well as computer usage necessary for other work duties.
  • Participate in individual and group clinical supervision in compliance with being an Adult Rule 79 Case Manager.
  • Provide leadership, coaching, and/or mentoring to a subordinate group as assigned.
