About The Position

Why M Physicians? The M Health Fairview Comprehensive Gender Care Program brings together specialists from across our system dedicated to providing compassionate and coordinated care for transgender, non-binary, and gender-diverse patients. We’re home to an interdisciplinary team of experts – ranging from primary care providers to mental health specialists and gender-affirming surgeons. What you will do as a Social Worker : Review Letters of Support and coordinate with perioperative coordinators to update clinicals for insurance coverage. Support patients and their family through the emotional and educational process of accessing gender affirming surgical care. Support patients’ navigation of resources and connection to healthcare services across the system and in the broader community, including but not limited to: identify and address gaps in care, support patient application for leave, write letters for legal name & gender change. Assist patients with counseling related to resources and case management. Assist patients in understanding the healthcare and managed care delivery system and act to resolve obstacles that prevent them from receiving adequate mental health care. Document appropriate information in the patient’s medical record to ensure communication of patient’s psychosocial needs for mental health and medical care. Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in health and behavior change. Support the team with the day-to-day management of complex patients and collaborate with patients’ providers across MHealth Fairview and beyond. Convene and facilitate regular meetings with providers across MHealth Fairview departments to coordinate patient care. Provide brief evidence-based supportive psychotherapy in individual or group modalities Individualizes plan of care to meet patient needs, partnering with patient/caregiver in decision making and education. Modifies interventions/service based on population served.

Requirements

  • Master's degree in Social Worker
  • Active MN LICSW License and eligibility to be credentialed and enrolled with payers
  • 2+ years of Social Work experience post-graduation
  • Some experience with Diagnostic Assessments
  • Some experience with adult and adolescent Transgender/Gender Diverse populations
