About The Position
The Licensed Social Worker implements resident services in accordance with regulations and established best practices. The role supports residents and families through advocacy, ongoing assessment and evaluation, service coordination, and care planning. Its primary focus is residents’ quality of life and helping them achieve optimal psychosocial, emotional, and behavioral well-being. The position reports to the Campus Director of Social Services.
Requirements
- Bachelor’s degree in Social Work, Gerontology, or a related field; an equivalent combination of professional experience and education may be considered at management’s discretion.
- Licensed by the Minnesota Board of Social Work and able to maintain unrestricted licensure.
- For Medicare Class A Hospice: CPR certification is required.
Nice To Haves
- One or more years of social work experience, preferably in a long-term care environment.
- For the Transitional Care Unit (TCU): one or more years of social work experience, preferably in a long-term care setting.
