Licensed Social Worker
About The Position
The Licensed Social Worker (LSW) assists with overseeing the planning, development, organization, implementation, evaluation, and direction of the facility’s Social Services Department in accordance with current federal, state, and local standards. The role works directly with discharge planners, families, or responsible parties to initiate facility tours and follow-up procedures. The LSW also supports the identification of patients’ and residents’ medically related social and emotional needs and helps maintain effective social service programs through coordination with facility personnel and outside community agencies.
Requirements
- Must have a current Social Work license in good standing with the state, as required.
- At least 3 years of experience in a long-term care facility.
- Ability to organize and prioritize many tasks effectively.
- Skilled communication, direction, and motivation abilities.
- Reliable transportation required.

- Bachelor’s degree in Social Work is strongly preferred.
Responsibilities
- Assist in planning, developing, organizing, implementing, evaluating, and directing the Social Services Department in accordance with current federal, state, and local standards.
- Develop and manage initial contact, as necessary, with discharge planners, families, or responsible parties to initiate facility tours and follow-up procedures.
- Assist in developing and implementing policies and procedures for identifying patients’ and residents’ medically related social and emotional needs.
- Maintain a good working rapport with facility personnel, other departments, and outside community health, welfare, and social agencies to support social service programs and meet patient and resident needs.
- Keep abreast of current federal and state regulations and professional standards.
