About The Position

St. Camillus is hiring a full-time Social Worker to join the Social Services Team in its Rehab Unit. The role conducts comprehensive assessments and coordinates services to address each resident’s psychosocial and mood state, behavioral needs, and discharge needs. It facilitates interdisciplinary coordination of resident needs and develops and implements safe, effective discharge plans. The position requires a strong skill set in discharge planning and coordinating external services for patients. Applicants with rehabilitation or sub-acute experience are encouraged to apply and will receive priority consideration.

Requirements

Bachelor’s degree in Social Work or a related field.
Minimum of 2 years’ experience in a health care setting; experience in a rehabilitation department is preferred.
Demonstrated ability to execute discharge planning efficiently in accordance with facility requirements.
Strong problem-solving and organizational skills.
Sound knowledge of community resources, entitlements, and insurers.
Strong crisis intervention skills.
Excellent verbal and written interpersonal skills that promote family engagement and collaboration across interdisciplinary teams.

Nice To Haves
