About The Position
Assesses psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. Provides discharge planning, arranges home health and DME, provides counseling, assists with placement/transfers, and provides referrals to community services.
Requirements
- Master's degree in Social Work (MSW) or equivalent
- Current Florida LCSW or equivalent licensure
- Minimum of 2 years of experience in a hospital or healthcare setting
- Experience with discharge planning, case management, and community resource referral
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Nice To Haves
- Experience in pediatric social work
- Knowledge of local community resources and services
- Bilingual in Spanish
