About The Position

ChristianaCare is seeking an energetic, motivated, and compassionate Social Worker who understands the importance of care coordination. The role is part of the Care Management Triad Team, which collaborates with an RN Case Manager and Utilization Management. The Social Worker resolves psychosocial barriers and supports patient discharge needs. The position is based in the Emergency Department in Newark and is listed for a 3:00 p.m.–11:30 p.m. shift. The successful candidate may be eligible for a $15,000 sign-on bonus, paid within the first year of a two-year commitment. ChristianaCare includes outpatient services, home health care, urgent care centers, three hospitals, a free-standing emergency department, a Level I trauma center, a Level III neonatal intensive care unit, a comprehensive stroke center, and regional centers of excellence in heart and vascular care, cancer care, and women’s health.

Requirements

Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited graduate school before the hire date.
Licensed CSW or MSW as required by the state of Delaware before the hire date.

Nice To Haves

Acute care social work experience preferred.
CCM certification preferred within 18 months of eligibility.
