About The Position

ChristianaCare is seeking an energetic, motivated, and compassionate Social Worker who understands the importance of care coordination. The role is part of ChristianaCare’s Care Management Triad Team Model, collaborating with an RN Case Manager, who manages patient care, patient progression, and discharge planning, and Utilization Management, which reviews patient status and anticipated payer coverage. The Social Worker resolves psychosocial barriers and supports discharge needs. The successful candidate may be eligible for a $15,000 sign-on bonus, paid within the first year of a two-year commitment.

Requirements

Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited graduate school before the hire date.
Licensed CSW or MSW as required by the state of Delaware before the hire date.

Nice To Haves

Acute Care Social Work experience.
CCM certification within 18 months of eligibility.
