About The Position

ChristianaCare is seeking an energetic, motivated, and compassionate Social Worker to support care coordination in the Emergency Department. The Social Worker works within ChristianaCare’s Care Management Triad Team Model alongside an RN Case Manager, who manages patient care, progression, and discharge planning, and Utilization Management, which reviews patient status for appropriateness and anticipated payer coverage. The Social Worker resolves psychosocial barriers and supports discharge needs. The role is based in Newark and is scheduled from 3:00 p.m. to 11:30 p.m. The successful candidate may be eligible for a $15,000 sign-on bonus, payable within the first year of a two-year commitment.

Requirements

A Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited graduate school, required before the hire date.
A Delaware state CSW or MSW license, as required by the state, before the hire date.

Nice To Haves

Acute care social work experience preferred.
CCM certification preferred within 18 months of eligibility.
