About The Position
ChristianaCare is seeking an energetic, motivated, and compassionate Social Worker to join their team. This role is crucial in resolving psycho-social barriers and supporting discharge needs within the Care Management Triad Team Model. The successful candidate will be part of an extensive network of healthcare services, including hospitals, emergency departments, and specialized care centers, all committed to Love and Excellence.
Requirements
- Master’s degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date.
- Licensed CSW or MSW as required by the state of Delaware prior to hire date.
Nice To Haves
- Acute Care Social Work experience preferred.
- CCM preferred within 18 months of eligibility.
Responsibilities
- Resolve psycho-social barriers.
- Support discharge needs.
- Collaborate within the Care Management Triad Team Model, working alongside RN Case Managers and Utilization Management.
