About The Position

The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. The position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction. All employees are responsible for extending the mission and values of Baltimore Washington Medical Center by dedicating oneself to providing the highest quality healthcare services to the communities we serve. Works collaboratively to maintain active communication with physicians, nurses, utilization review staff, and other members of the multidisciplinary team to effect timely, appropriate patient management through the healthcare system. Makes recommendations for potential alternatives to care plan based on availability of resources and patient/family choices. Utilizes problem solving skills and negotiating techniques to resolve conflicts in patient care planning and delivery. Applies knowledge of age-specific, cultural diversity, psycho/social and developmental issues during the interview process, documentation, and interventions with the patient/family. Actively engages members of the healthcare team in discussion re: patient plan and progression through the healthcare system through interdisciplinary rounds, telephone calls, and face to face communication. Follows up with staff members to assure that the recommended plan of action is implemented once decisions are made. Provides support to patients/families requiring assistance with advanced directives, guardianship, homelessness, and connection/referrals to support agencies for crisis intervention. Formulates unique solutions to complex discharge planning issues (“thinking out of the box”).

Requirements

  • Graduation from an accredited School of Nursing or college with current nursing licensure in the State of Maryland.
  • Minimum of 3-5 years of relevant clinical hospital experience required as applicable to the position needs.
  • Master’s degree in Social Work accredited by Council on Social Work Education (CSWE).
  • LMSW (Licensed Master Social Worker) licensure from the Maryland Board of Social Work Examiners.
  • Minimum one (1) year of related experience is required.
  • Demonstrate skill in general social work and/or case management practice areas.
  • Demonstrate skill in completing social work functions under supervision, including biopsychosocial assessment, resource coordination, case management, psychoeducation, referral, information gathering, therapy, counseling, mental health diagnosis, consultation, advocacy.
  • Demonstrate effective critical thinking.
  • Demonstrate effective written and oral communications.
  • Demonstrate age-appropriate interpersonal interactions (patients may range from newborn to geriatric adult).
  • Ability to communicate and collaborate effectively with both internal and external stakeholders (i.e. colleagues, medical staff, liaisons, patient, family).
  • Ability to assess, adapt, and calmly respond to changing and/or crisis environment.
  • Ability to make decisions consistent with current policies, procedures, and ethical standards while operating under supervision.
  • Ability to prioritize work assignments and manage time effectively to complete duties.
  • Ability to understand and follow written and verbal instruction.
  • Adhere to the professional principles and standards of practice consistent with underlying core values of the profession, as outlined by the NASW Code of Ethics.
