About The Position

Givens Highland Farms, a continuing care retirement community in Black Mountain, is hiring a full-time Health Services Social Worker. The role provides social services navigation and support for the Health Care Center (Skilled Nursing/Long Term Care) and Residential Assisted Living (Multi-unit Assisted Housing with Services). The Social Worker works to support residents and make a direct impact on their lives.

The position serves as the social services point of contact for both settings, collaborating with health services leadership and the interdisciplinary Care Plan Team to provide comprehensive, individualized resident care. It includes psychosocial assessment and care planning, resident advocacy, discharge planning and care transitions, and support with admissions.

Requirements

• Master of Social Work (MSW).
• At least 2 years of experience working with older adults.
• Valid North Carolina driver’s license with a clear driving record for use of the facility vehicle.

Responsibilities

• Serve as the social services point of contact for the Health Care Center and Residential Assisted Living.
• Collaborate with the Health Services Director, Director of Nursing, and interdisciplinary Care Plan Team to ensure comprehensive and individualized resident care and services.
• Complete comprehensive psychosocial assessments and develop, implement, and monitor individualized care plans for residents in both settings.
• Advocate for and uphold Residents’ Rights, including documenting and following up on resident and/or family complaints or grievances.
• Coordinate discharge planning and care transitions with residents, families, the interdisciplinary Care Plan Team, and external agencies.
• Assist with coordinating admissions and serve as backup to the admissions coordinator as needed.
• Perform other duties and tasks assigned on an as-needed basis by an authorized supervisor.
