Requirements

  • Bachelor’s degree in Social Work, Master’s degree preferred. Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
  • Licensure and/or certification as required by the State.
  • Minimum of 2+ years social service experience in a health care setting or social services agency.
  • Deep comfort with grief, loss, and end-of-life conversations
  • Strong knowledge of community resources and care coordination
  • Excellent documentation skills and attention to compliance
  • Self-directed with the ability to work with little supervision
  • Valid driver's license and willingness to travel locally for patient visits

Nice To Haves

  • Experience in hospice, palliative care, or a related healthcare setting preferred

Responsibilities

  • Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
  • Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
  • Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
  • Guiding families through Advanced Directives with clarity and sensitivity
  • Assisting with facility placement for patients when appropriate
  • Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
  • Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
  • Participating in on-call duties on evenings and weekends as needed
  • Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures
