Hospice Social Worker
About The Position
Benaroya Services at Kline Galland is looking to immediately hire a Full-Time Social Worker to join our amazing Hospice and Palliative Care team. This is a meaningful opportunity to make a difference by supporting patients and families during some of life’s most important moments. The Social Worker will provide a range of essential services to support the emotional, psychological, and social needs of patients and families receiving hospice and palliative care. Provide care across multiple settings, including: Private residences, Adult Family Homes, Independent & Assisted Living communities, Skilled Nursing Facilities.
Requirements
- Master’s Degree MSW from an accredited program
- Minimum 2 years of social work experience
- Proficiency in computer charting and documentation systems
- Valid driver’s license, auto insurance, and reliable transportation
Nice To Haves
- 1 year in Hospice and/or Home Health preferred
- Strong verbal and written communication skills
Responsibilities
- Conduct comprehensive psychosocial assessments
- Develop and implement plans of care in collaboration with the interdisciplinary team
- Provide crisis intervention and therapeutic support
- Coordinate resource referrals and community services
- Support patients and families navigating end-of-life care
