Requirements

  • Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
  • Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.

Nice To Haves

  • Hospice or palliative care experience, preferred.
  • Two (2) years of verifiable experience working as a Social Worker, preferred.

Responsibilities

  • Assess patients’ psychosocial needs and complete social histories
  • Document care plans, progress, and follow-up per regulations
  • Provide counseling and emotional support to residents and families
  • Support patients and families with adjustment, rights, and orientation
  • Coordinate referrals and act as a liaison with community resources
  • Partner with the care team to enhance quality of life and outcomes
  • Plan and support patient, family, and community engagement activities
  • Promote facility programs and services to referral partners
