About The Position

Andwell Health Partners is seeking a licensed LCSW to provide clinical leadership for its Community Care Team (CCT) and H.O.M.E. programs. The role oversees patient assessments, care planning, clinical supervision, documentation quality, and regulatory compliance while supporting a multidisciplinary care management team. The position maintains a small caseload and provides coverage as needed. It is full-time, 40 hours per week, based in Lewiston, Maine, with a hybrid work opportunity. The stated salary range is $67,080 to $106,787 per year.

Requirements

• Master's degree in Social Work, Counseling, or a related field.
• Current Maine licensure as an LCSW.
• Strong leadership, clinical assessment, and care coordination skills.

• Previous supervisory or team leadership experience.

Responsibilities

• Supervise, mentor, and support case management staff.
• Review and sign off on assessments and care plans completed by case managers.
• Review and approve H.O.M.E. patient care plans.
• Audit EHR documentation to ensure quality and compliance.
• Coordinate care with providers, community partners, and families.
• Support onboarding, training, and clinical consultation.
• Maintain a small caseload and provide coverage as needed.
• Ensure compliance with HIPAA, MaineCare, and organizational standards.
