About The Position

Henry Ford Health’s Community Care Services (CCS) team expands access to care through community-based programs, connecting individuals with health resources, support services, care coordination, home health care, and hospice services. The Social Worker-CCS provides social work services to patients and families, addressing emotional, social, and practical challenges and supporting patients throughout their healthcare journey. The role focuses on comprehensive care that considers each patient’s individual needs and circumstances. The position involves assessing and intervening in situations involving illness, handicaps, death, and loss to improve patient care and staff effectiveness. The social worker applies knowledge of medical procedures, terminology, etiology, and disease processes and collaborates with healthcare teams to develop and implement holistic care plans. Requirements

Master of Social Work degree.
Licensed Master's Social Worker (LMSW) in the State of Michigan.

Nice To Haves

ACSW certification.
Previous experience in a healthcare setting.
