About The Position

The Clinical Social Worker I/II (CSW I/II) works under the general supervision of the Clinical Supervisor of the Roving Team, providing in-person outreach at single room occupancy hotels and permanent supportive housing sites in San Francisco’s Tenderloin, South of Market, and Mission neighborhoods. These sites house formerly homeless adults placed through city subsidies. The clinician engages high-risk clients with mental illness, complex medical needs, and/or substance use concerns to help improve housing retention and reduce eviction and returns to homelessness by building relationships and connecting clients with needed supports. The role carries a caseload and provides community-based clinical case management, including engagement, assessment, treatment planning, psychotherapy, crisis intervention, 5150 assessments and holds, substance use treatment, psychoeducation, referrals, advocacy, family communication, mandated reporting, and coordination with housing staff and community providers. The clinician coordinates with acute hospitals and outpatient medical providers to support continuity of care, and documents interventions, assessments, and referrals in Epic or other electronic medical records. The position participates in a weekly two-hour clinical team meeting for training, resource sharing, peer support, and case consultation, as well as one hour of weekly clinical supervision. The work requires lifting up to 50 pounds, walking several miles a day, and climbing stairs. The Department of Psychiatry at ZSFG integrates medical, psychiatric, and addiction treatment services and is San Francisco’s largest provider of acute psychiatric care and its only provider of psychiatric emergency care.
