About The Position

Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II conducts in-person outreach at permanent supportive housing sites, including single-room-occupancy hotels in San Francisco’s Tenderloin, South of Market, and Mission neighborhoods. These sites house formerly homeless adults placed through city housing subsidies. The role engages high-risk clients with mental illness, complex medical needs, and/or substance use to support housing retention and reduce the risk of eviction and a return to homelessness by building relationships and connecting clients with needed services. The clinician maintains a caseload and provides community-based clinical case management, including engagement, assessment, treatment planning, psychotherapy, crisis intervention, 5150 assessments and holds, substance-use treatment, psychoeducation, referrals, coordination with property management and housing-site support staff, advocacy, family communication, and mandated reporting as needed. The clinician coordinates and consults with acute hospitals and outpatient medical providers to improve continuity of care. Interventions, assessments, and referrals are documented promptly in Epic or other electronic medical records. Other duties may be assigned. The role requires a basic understanding of DSM-5 diagnostic criteria, harm-reduction principles, trauma-informed care, and anti-racism. The clinician works with diverse and multicultural teams, clients, and communities, contributes to culturally focused clinical interventions, and practices cultural humility and anti-racism. The Supportive Housing Teams hold a two-hour weekly clinical team meeting for training, resource sharing, peer support, and case consultation, and each clinician receives one hour of clinical supervision weekly. The position is in the Department of Psychiatry at ZSFG, which integrates medical, psychiatric, and addiction treatment services. The work involves lifting up to 50 pounds, walking several miles per day, and climbing stairs.
