Requirements

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

• Clinical Counselor Preferred.
• Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
• Previous case management or discharge planning experience
• MSW

Responsibilities

• Lead the interdisciplinary plan of care from admission to discharge, ensuring all strategies integrate the patient’s personal values and preferences across the entire health continuum.
• Serve as the primary resource for assessing and treating complex psychosocial needs, including crisis intervention, substance abuse, neglect, and legal or adjustment challenges.
• Proactively identify and remove barriers to recovery by advocating for essential resources and navigating the complexities of social and medical systems on behalf of the patient.
• Maintain a continuous feedback loop with the care transition team to reevaluate health plans and ensure all medical documentation accurately reflects the complexity of the patient's treatment and post-discharge needs.
