About The Position

This is a temporary, grant-funded position lasting at least 3 years. It is a staff position with the TIEH Treatment of Individuals Experiencing Homelessness (TIEH) program and is funded through Pinellas County Health and Human Services. The TIEH program is a collaborative effort by Boley Centers, Inc., the Pinellas County Sheriff’s Office (PCSO), Pinellas County and Eleos (formerly PEMHS), and is located at the PCSO Emergency Shelter (Safe Harbor). The TIEH Case Manager assists clients in securing appropriate services to include vocational, medical, and community supports and services.

Requirements

  • Bachelor's degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and at least one year of full time experience working with adults experiencing serious mental illness.
  • Or a bachelor's degree and three years full time or equivalent experience working with adults experiencing serious mental illness.
  • Education must be from an accredited school, college, or university.
  • Knowledge of mental illness and symptoms, mental health system and supports and community social services systems.
  • Valid Florida driver's license, own transportation vehicle and vehicle insurance required.
  • Must pass a level II background check and drug screen.

Responsibilities

  • Performs client intake, admission and discharge to and from the program.
  • Completes assessments of clients’ medical/dental health, use of drugs or alcohol, education, employment, social development, activities of daily living, family structure and Baker Act and incarceration histories.
  • Conducts assessments to determine strengths, needs and preferences.
  • Maintains caseload and provides direct services including biopsychosocial assessment, recovery planning, case management and housing support.
  • Works with clients to develop comprehensive service plans to initiate housing referrals and connections. Also implements service plans for each assigned client and updates plans monthly.
  • Coordinates in-house and/or community appointments, i.e. medical, dental, therapy, etc.
  • Makes and/or receives calls regarding a broad spectrum of client related issues; involves contact with a wide variety of individuals (parents, friends, etc.), social services (Medicaid, Social Security, etc.) and service providers, both internal and external.
  • Develops and monitors safety plans.
  • Maintains working relationships with local service and treatment providers to ensure clients’ access to services and treatment.
  • Completes required documentation to ensure ongoing compliance with all internal and external policies and procedures.
  • Maintains ongoing awareness of each assigned clients’ needs by frequently reviewing documentation including activity reports, progress notes, etc. and by frequently meeting with each client.
  • Operates own or agency vehicle to transport clients and/or travel between locations.
