Social Worker - Case Manager
About The Position
The Social Worker – Acute Care identifies patients with complex psychosocial, financial, placement, and community-service needs within 24 hours of admission. The role assesses barriers to discharge, develops individualized discharge plans, and coordinates with patients, families, physicians, RN Care Managers, payers, and community agencies to facilitate safe, timely transitions to the next level of care. Additional duties include readmission risk assessment, family care conferences, resource referrals, insurance benefit review, EMR documentation, and reporting discharge delays and avoidable days.
Requirements
- Licensed Master’s Social Worker (LMSW) is required; obtain LMSW certification within one year of eligibility and maintain continuing education requirements.
- Acute Care experience is required.
- Licensed Bachelor Social Worker (LBSW) is required; obtain LBSW certification within one year of eligibility and maintain continuing education requirements.
Nice To Haves
- Three years of acute hospital care or social work experience.
- BLS certification from the American Heart Association, American Red Cross, or equivalent through the Military Training Network (MTN).
