About The Position

Pathways, Inc. is seeking a compassionate, organized, and recovery-oriented Social Worker/Case Manager to provide comprehensive case management, counseling, advocacy, and supportive services to adults living with chronic and severe mental illness. The Social Worker/Case Manager serves as the primary coordinator of care, helping clients achieve greater independence, stability, and quality of life through individualized treatment planning and community-based support services.

Requirements

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field required.
  • Minimum of two (2) years of experience providing case management or behavioral health services to individuals with serious mental illness.
  • Valid Connecticut driver's license with an acceptable driving record.
  • Reliable transportation and ability to transport clients when necessary.
  • Knowledge of mental health treatment, recovery-oriented practices, and case management principles.
  • Understanding of community resources and federal, state, and local entitlement programs.
  • Strong assessment, problem-solving, and crisis intervention skills.
  • Excellent verbal, written, and interpersonal communication skills.
  • Strong organizational and time-management abilities.
  • Ability to work independently while functioning effectively within a multidisciplinary team.
  • Proficiency in Microsoft Office 365 and Electronic Health Record (EHR) systems.
  • Commitment to person-centered care, cultural competency, and professional ethics.

Nice To Haves

  • Master's Degree in Social Work (MSW) or related behavioral health field preferred.
  • Experience working in community mental health, residential services, supportive housing, or related settings preferred.

Responsibilities

  • Conduct psychosocial assessments and develop individualized treatment and service plans.
  • Provide case management, supportive counseling, crisis intervention, and advocacy services.
  • Coordinate medical, psychiatric, housing, vocational, financial, and community support services.
  • Assist clients with activities of daily living, benefits management, appointment scheduling, transportation planning, and community integration.
  • Monitor client progress and adjust service plans as needed to support recovery goals.
  • Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR) system.
  • Complete all required state, agency, contractual, and accreditation documentation.
  • Participate in interdisciplinary team meetings, case conferences, and treatment planning sessions.
  • Provide support during client admissions, transitions, and discharges.
  • Collaborate with families, conservators, healthcare providers, and community partners to ensure continuity of care.
  • Participate in on-call coverage as assigned.
  • Adhere to all agency policies, procedures, confidentiality standards, and professional ethics.
