About The Position

VIVA HEALTH is seeking a Social Worker, Case Manager to join their team. This role is responsible for assessing the social, emotional, and environmental needs of Alabama Coordinated Health Network (ACHN) patients to improve medical compliance and health outcomes. The position identifies barriers to medical compliance, such as lack of transportation or illiteracy, and ensures services are delivered and continuity of care is maintained. The individual analyzes the home and community environment, makes autonomous decisions regarding patient-centered care plans and goals, and provides services primarily in community and home settings via phone and/or in person. Local daytime travel is required. This position will have work-from-home opportunities.

Requirements

  • Licensed BSW
  • Current SW license in good standing with the appropriate State of Alabama board
  • Valid driver's license in good standing
  • One to three years in a community-based clinical role
  • Excellent interview and telephone skills
  • Ability to deal with recipients in a caring and helpful manner
  • Working knowledge of health-related service delivery systems
  • Excellent communication and relationship skills
  • Ability to analyze varied environmental factors to members' well-being
  • Ability to work independently in an autonomous setting
  • Ability to locate, augment, and develop resources, including information on services offered by other agencies.
  • Willingness to submit to vaccine testing and screening
  • May require significant face-to-face member contact with duties performed away from the principal place of business.

Responsibilities

  • Identification of barriers to medical compliance such as lack of transportation, illiteracy, or other social determinants that impact a patient's health.
  • Ensuring services are delivered and continuity of care is maintained.
  • Analysis of the home and community environment.
  • Making autonomous decisions regarding the development of appropriate patient centered care plans and goals.
  • Providing services primarily in community and home settings via phone and/or in person.
  • Local daytime travel is required via a reliable means of transportation insured following Company policy.
