About The Position

This role is for a Social Worker Case Manager in the Pediatric Psychiatry department at Ascension St. Francis Hospital in Milwaukee, WI. The position is part-time and operates on a day schedule. The social worker will be responsible for designing personalized care plans, delivering expert psychosocial interventions, breaking down care barriers by advocating for community resources, and guiding post-discharge transition planning in consultation with multidisciplinary teams.

Requirements

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Responsibilities

Design personalized care plans: Partner with patients and families to build holistic continuum-of-care strategies that align medical goals with their unique personal values and preferences.
Deliver expert psychosocial interventions: Evaluate and support complex patient needs—including crisis management, substance abuse, and legal or adjustment challenges—to guide families through difficult transitions.
Break down care barriers: Advocate relentlessly for necessary community resources and eliminate social determinants of health to ensure equitable, uninterrupted care.
Guide post-discharge transition planning: Consult with multidisciplinary teams to ensure precise medical documentation, recommend appropriate care levels, and secure safe post-acute services.
