About The Position

This role focuses on designing personalized care plans, delivering expert psychosocial interventions, breaking down care barriers, and guiding post-discharge transition planning. The Social Worker Case Manager will partner with patients and families to build holistic continuum-of-care strategies, evaluate and support complex patient needs including crisis management, substance abuse, and legal or adjustment challenges, advocate for necessary community resources, and consult with multidisciplinary teams to ensure precise medical documentation and secure safe post-acute services.

Requirements

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Responsibilities
