About The Position

The Social Worker Case Management position is located in the Emergency Department and Psych ED at Dell Seton Medical Center in Austin, TX. This role involves rotating shifts, including overnight coverage on weeknights and weekends. The primary focus is on providing comprehensive social work services within the emergency department setting. The social worker will be responsible for discharge planning, connecting patients with community resources, and coordinating placements in psychiatric facilities. They will respond to a wide range of critical situations such as suicidal ideation, psychosis, substance abuse, domestic violence, sexual assault, and homelessness. Duties also include identifying and notifying next of kin for trauma patients and offering support in cases involving death. The role emphasizes leading the design and execution of personalized care plans, advocating for patients by removing barriers to care, and supporting families through complex psychosocial challenges. Collaboration with the interdisciplinary care team and community agencies is essential for refining treatment strategies and ensuring high-quality medical documentation.

Requirements

Bachelor's degree in Social Work required
Active State Licensure: Current and unrestricted license in Social Work (LMSW/LCSW) as required by state regulations
Clinical Case Management Experience: A proven track record of coordinating complex discharges and care transitions within an acute care or clinical setting
One or more of the following credentials obtained prior to hire date or job transfer date: Clinical Counselor, Counselor credentialed from the Texas State Board of Examiners of Professional Counselors, Social Worker credentialed from the Texas State Board of Social Worker Examiners, Social Worker specializing in Clinical Practice

Nice To Haves

3 years of experience preferred
Case Manager credentialed from the Commission for Case Manager Certification (CCMC)
Case Manager credentialed from the American Case Management Association (ACMA)
