We are seeking a Social Worker – Emergency Department to provide psychosocial assessment, intervention and support to Emergency Department patients and their social supports. This role involves providing response and intervention in the context of Trauma activations, acute grief support, assisting with psychiatric dispositions, and supporting patient needs such as transportation, mental health, housing/homelessness, in-home support, hospice, and insurance programs. The Social Worker will also serve as a support to patients, families, and the ED team in the context of abuse and neglect concerns, and connect patients with community programs and resources to support resolution of psychosocial risks and barriers to discharge and overall health and wellbeing. Additionally, the role requires intervening with support systems for patients with complex medical needs, providing support, crisis management, and education. The Social Worker will support the Social Work team through case consultation, coverage support, and participation in improvement projects. There is an option to participate in clinical supervision to advance clinical licensure and in Social Work staffing models to address needs across the care continuum. The position utilizes Social Work interventions to enhance patient and family functioning and collaborates with the multidisciplinary Emergency Department team to develop patient goals, monitor progress, address barriers, and evaluate outcomes.
