About The Position

This is a full-time Social Worker Case Management position. The role involves providing psychosocial assessments, utilizing brief interventions and supportive counseling techniques, and collaborating with various disciplines within the hospital and community. The Social Worker will also provide crisis intervention, consider the needs of different age groups, understand the continuum of care in the healthcare system, and interpret this system to patients, families, and the healthcare team. Additionally, the role requires understanding third-party payor systems related to discharge planning and utilizing basic computer skills. Requirements

  • BS in health-related field
  • Graduate curriculum (leading to a master’s degree) in an accredited school of social work preferred or has the documented equivalent in education, training, and/or experience
  • Social work experience in acute care setting preferred

Responsibilities

  • Provides psychosocial assessments on an as-needed basis, and in accordance with Case Management Department Policies and Procedures.

  • Utilizing brief interventions/supportive solutions-focused counseling techniques for identified issues.
  • Working collaboratively with multiple disciplines within the hospital setting and community.
  • Providing crisis intervention whenever appropriate.
  • Considering the special needs and behaviors of specific age groups.
  • Able to understand the continuum of care in the health care system and possess the communication skills to interpret the system to patients, families, and other members of the health care team.
  • Able to understand third-party payor systems as they relate to discharge planning.
  • Utilizing basic computer skills.
