About The Position

Regional West Health Services is seeking a compassionate and dedicated Medical Social Services Representative or Social Worker (BSW) to join our team. In this role, you will provide psychosocial support, discharge planning, case management, advocacy, and resource coordination for patients and families across the continuum of care. This position plays a vital role in helping patients navigate healthcare challenges while promoting safety, independence, and optimal outcomes.

Requirements

• Social Worker (BSW): Bachelor's Degree in Social Work from a CSWE-accredited program.
• Social Worker (BSW): Certified Social Worker through the Nebraska Department of Health.
• Medical Social Services Representative: Bachelor's degree in a human services-related field.
• Medical Social Services Representative: Minimum of two years of human services experience required.
• Excellent communication and interpersonal skills.
• Strong assessment, problem-solving, and organizational abilities.
• A passion for helping patients and families navigate complex healthcare situations.
• The ability to work collaboratively with multidisciplinary teams.
• Commitment to compassionate, patient-centered care.

Nice To Haves

• Healthcare or hospital social work experience preferred.

Responsibilities

• Provide psychosocial support, discharge planning, case management, advocacy, and resource coordination for patients and families.
• Conduct comprehensive assessments to identify psychosocial, emotional, and discharge planning needs.
• Develop discharge plans and coordinate care with patients, families, physicians, nurses, therapists, and community partners.
• Arrange post-hospital services and secure community resources.
• Educate patients and families about available resources and connect them with programs and services.
• Support patients and families through new diagnoses, life-changing events, or unexpected crises.
• Communicate regularly with healthcare providers, referral agencies, and community organizations to ensure coordinated, comprehensive support.
• Maintain accurate documentation of assessments, interventions, care plans, and patient progress.
• Assist with sensitive situations involving abuse or neglect, serving as a resource and coordinating appropriate reporting and support services.
