About The Position
Regional West Health Services is seeking a compassionate and dedicated Medical Social Services Representative or Social Worker (BSW) to join our team. In this role, you will provide psychosocial support, discharge planning, case management, advocacy, and resource coordination for patients and families across the continuum of care. This position plays a vital role in helping patients navigate healthcare challenges while promoting safety, independence, and optimal outcomes. A Day in the Life of No two days are exactly alike, but every day begins with a common purpose: helping patients and families navigate complex healthcare situations and ensuring they have the support they need to achieve the best possible outcomes. Your day may start by meeting with newly admitted patients and conducting comprehensive assessments to identify psychosocial, emotional, and discharge planning needs. As you learn each patient's unique circumstances, you begin building a plan that supports both their medical and social needs. Throughout the day, you'll work closely with patients, families, physicians, nurses, therapists, and community partners to develop discharge plans and coordinate care. Whether arranging post-hospital services, securing community resources, or helping a family understand available options, you'll play a key role in ensuring a safe and successful transition of care. As a trusted advocate, you'll help patients and families navigate difficult decisions while ensuring their rights, choices, and safety remain at the center of every care plan. You'll provide education about resources and connect individuals with programs and services that can make a meaningful difference in their lives. Some moments are especially impactful. You may find yourself supporting a patient and family through a new diagnosis, a life-changing event, or an unexpected crisis. During these times, your compassion, guidance, and ability to listen help families build coping skills and make informed decisions about their care. Collaboration is a cornerstone of the role. You'll communicate regularly with healthcare providers, referral agencies, and community organizations to ensure patients receive coordinated, comprehensive support throughout their healthcare journey. You'll also maintain accurate documentation of assessments, interventions, care plans, and patient progress, helping ensure continuity of care across the healthcare team. At times, you'll assist with sensitive situations involving abuse or neglect, serving as a resource and coordinating appropriate reporting and support services to protect vulnerable patients and families. At the end of the day, you'll leave knowing you've made a real difference by helping patients overcome barriers, access resources, navigate challenging circumstances, and move forward with confidence and support.
Requirements
- Bachelor's Degree in Social Work from a CSWE-accredited program (for Social Worker role)
- Certified Social Worker through the Nebraska Department of Health (for Social Worker role)
- Bachelor's degree in a human services-related field (for Medical Social Services Representative role)
- Minimum of two years of human services experience required (for Medical Social Services Representative role)
- Excellent communication and interpersonal skills
- Strong assessment, problem-solving, and organizational abilities
- The ability to work collaboratively with multidisciplinary teams
- Commitment to compassionate, patient-centered care
- Successfully passing a pre-employment drug test and background check
- A Physical Capacity Profile may be required for some positions
