Requirements

• Bachelor's Degree Social Work.
• 1 year Work or practicum experience in the provision of social services.

Responsibilities

• Resources, advocacy and referrals.
• Assists patients and families to navigate systems of care utilizing personal, community and governmental resources.
• Addresses the comprehensive impact of health conditions on the client and family in partnership with medical providers to meet their various social service concerns.
• Works in support of the Social Services Clinician: MSW, LMSW, LCSW.
