About The Position

The Medical Social Worker (BSW) provides social work services to patients and families receiving skilled care from the Home Health Agency. Under the supervision of a qualified Master of Social Work (MSW), the BSW supports the social, emotional, and resource needs of clients coping with acute or chronic illness in the home setting. This position complies with Pennsylvania Department of Health regulations for licensed home health agencies and the Medicare Conditions of Participation. Requirements

  • Bachelor of Social Work (BSW) from an accredited institution.
  • Must receive regular supervision by a qualified MSW (per PA regulations).
  • Current certification in Basic Life Support (BLS/CPR).
  • Valid PA driver’s license, access to reliable transportation, and proof of auto insurance.
  • Must pass criminal background check and other required clearances.

Nice To Haves

  • Minimum of one (1) year of experience in a healthcare or home health setting.
  • Social work license in Pennsylvania preferred but not required at the BSW level.

Responsibilities

  • Conduct psychosocial assessments under MSW supervision.
  • Assist in developing individualized care plans that address the patient’s social and emotional needs.
  • Provide short-term counseling, education, and supportive interventions to patients and families.
  • Facilitate access to community resources, financial aid, and long-term care planning.
  • Educate and assist patients with advance directives and healthcare planning.
  • Support care coordination, including transitions to other levels of care (e.g., inpatient, skilled nursing, respite).
  • Collaborate with the interdisciplinary care team and participate in case conferences.
  • Document all services per agency, state, and federal guidelines.
  • Maintain an organized and efficient caseload and visit schedule.
  • Demonstrate understanding of home health billing and compliance.
  • Maintain accurate and timely clinical documentation.
  • Participate in quality improvement activities as requested.
  • Adhere to policies and procedures, and uphold ethical and professional standards.
  • Demonstrate empathy, professionalism, and respect in all interactions.
  • Respond to patient/family needs and concerns promptly and compassionately.
  • Support agency efforts to deliver high-quality, patient-centered care.
  • Participate in audits and chart reviews as requested.
  • Contribute to departmental and agency-wide improvement initiatives.
  • Engage in training, in-services, and educational activities to enhance performance.
  • Attend agency meetings, in-services, and staff development activities.
  • Participate in community outreach and educational sessions when assigned.
  • Perform other related duties as required.
