About The Position
The Inpatient Bachelor of Social Work (BSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management BSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access. Position is full-time, 1.0 FTE, Day Shift, 40 hours per week. $10,000 Hiring Bonus for eligible external hires that meet required qualifications and conditions for payment. Requirements
- Bachelor's Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
- Upon hire: Alaska Baccalaureate Social Worker License
- 1 year SW experience in any healthcare setting
- Recent experience in a medical facility or long term care setting.
Nice To Haves
- National Certification in area of specialty
- 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program
- Basic understanding of acute medical surgical healthcare terminology
- Prior case management experience.
